Access the Guide Path Community Portal and its entire suite of tools, resources and support materials to help you understand the unique needs and expectations of your resident and family members, while identifying areas of potential risk.

  • Distribute the Guide Path Resident and Family Insights Survey assessment tool.
    • Email the survey directly through the portal to the resident/family (or download/print a hard copy to complete with the resident/family in-person).
    • Review the individualized insights data and survey responses you receive.
    • For each completed RFI Survey, a risk rating is calculated for each of the 5 Areas of Interest in addition to an overall aggregated risk-rating. Use the easy-to-read Guide Path Insights Survey Risk Rating (identified by High, Medium, or Low risk) to understand where any potential risks may exist.
  • Use the Guide Path Insights Survey High-Risk Roadmap, which provides proactive, evidence-based actions that your community staff can immediately implement when medium- or high-risks are identified.
  • Share the Guide Path Resident and Family Education Videos that your community can provide to family members to explain the RFI Survey to help set realistic expectations for transitioning care to your care community.

Once your community purchases the Survey Suite, you will receive details about access to the members-only dashboard and resource collective.

2. What benefits do I get as a member, and are there different membership tiers?For a limited time, your community’s membership guarantees lifetime access to the Survey Suite and all resources. There is only one membership type. In the future, the Survey Suite will offer several membership options with different valuable resources based on the membership level.


3. Can I share my membership login credentials with others?

The community will have unique credentials with secured access, which cannot be shared with other communities.


4. How many surveys can I generate?

Good news! With your membership, your community can create an unlimited number of surveys and risk ratings.


5. Are there any limitations on downloading or saving membership-only materials?

While there are limitations on some protected materials and resources, they are designed to be used and shared for resident and family and community engagement and proactive expectations and risk management.


6. What payment methods are accepted for membership subscriptions?

Payments can be made in the form of a credit card or ACH.


7. Is there a refund policy for membership subscriptions if the community is unsatisfied?

Guide Path’s customer service model is the gold standard. If your community is not satisfied with the Survey Suite and resources, a 50% refund will be offered. Due to administrative expenses, a full refund is not offered.


8. How often is new content added for members, and can I suggest topics or materials?

Guide Path plans to add new content and risk management resources and programs continuously. We want your feedback, ideas and suggestions! Guide Path is YOUR community!
